FREE Integrated Care for S.T.A.R. members with no insurance or those on any of the 7 AHCCCS Complete Care Plans. Ongoing monitoring and treatment for: diabetes, high blood pressure, COPD and weight management. Ongoing education on living with these conditions. Preventative care including: smoking cessation, flu and pneumonia vaccines, well-woman and well-man exams, and over-the-counter medications. Access to prescriber, medical evaluations, medical prescriptions and labs at no cost.
